Director- Case Management

Under the general direction of the Vice President of Quality and Performance Excellence, is responsible for program planning, oversight, and evaluation for the Community Hospital Division Case Management department. Oversees and provides direction to the Case Management department in the provision of utilization review, discharge planning, and social services, and the implementation of the Clinical Documentation Improvement program. Is responsible for program planning and development and oversees implementation of the Palliative Care Program. Collaborates with and assists the Utilization Review Medical Director in facilitating appropriate utilization of hospital resources, and facilitates improvement teams and projects to meet hospital outcomes aligned with Froedtert Health strategic goals and the Value Based Cost Structure. Assures departmental compliance with state and federal regulations and contributes to third-party payer negotiations regarding protocols and operations. Other duties as assigned.
 
  • At least 8 years of experience in a hospital clinical setting
  • At least 3 years relevant leadership/management experience
  • 5 years of hospital case management experience   
  • Utilization management experience is preferred
  • Bachelor’s Degree in Nursing or Master’s Degree in Social Work is required
  • Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact) or Wisconsin Licensed Social Worker
